On this 3rd day of October A. D. one thousand eight hundred and ninety two personally appeared before me, a notary public of the [blank] Court, a Court of Record within and for the County and State aforesaid Alexander F. McFann aged 45 years, a resident of the township of Ch[?] County of McHenry, State of Illinois, who, being duly sworn according to law, declares that he is the identical person who was ENROLLED on the first day of November, 1864 in 1st Battery of Indiana light artillery in the war of the rebellion, and he served at least ninety days, and was HONORABLY DISCHARGED at Indianapolis Ind, on the 22nd day of August, 1865. That he is now unable to earn a support by reason of disease of eyes [and] kidney trouble and catarrh and disease of the throat. That said disabilities are not due to his vicious habits, and are to the best of his knowledge and belief permanent. That he has [blank] applied for pension under application No. [blank]. That he is a pensioner under Certificate No [blank]. That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the Act of June 27, 1890. He hereby appoints M. K. Martin2 aged 48 years, a resident of Libery Mills in the County of Wabash and State of Indiana whose Post Office is Liberty Mills and Joe Cargill aged 43 years, a resident of North Manchester in the County of Wabash and State of Indiana whose Post Office is North Manchester well known to me to be reputable and entitled to credit, and who, being duly sworn, declared in relation to aforesaid case as follows: that Alexander McFann and Franklin McFann is the one and the same person -- we obtain our information from a long acquaintance and being members of the same Co and Regt with the claimant.

McFann aged 49 years, a resident of the State of Indiana, who being duly sworn according to law, declares that he is the identical person who was ENROLLED on the 12th/also 1st day of May/Novr, 1864/1864 in Private, Co G. 138 Regt Ind vol/1st Batty of Ind Artillery in the war of the rebellion, and served at least ninety days, and was HONORABLY DISCHARGED at Indianapolis Ind/Indianapolis Ind, on the 22/22 day of Sept/Augt, 1864/1865. [The person who filled out this form entered the information on McFann's two enlistments by writing the information on his second enlistment above the information on his first enlistment. I have represented this way of filling out the form by writing a/b where b, refering to his second enlistment, was in the original written above a, refering to his first enlistment. Since it is difficult to clearly represent the way the information was entered on this form, here is a summary of his enrollment information: he was enrolled on 12 May 1864 in Co G. 138th Regiment of Indiana volunteers and honorably discharged at Indianapolis on 22 September 1864. He then enrolled 1 November 1864 in the 1st Battery of Indiana Artillery and was honorably discharged at Indianapolis on 22 August 1865.]

McFann aged 50 years, a resident of South Bend in the County of St Joe and State of Indiana whose Post-Office address is South Bend Ind well known to me to be reputable and entitled to credit, and who, being duly sworn, declares in relation to the aforesaid case as follows: that he was a member of Co G. 138 Regt Ind vol infty --- was enrolled May 12 -- 1864. Was discharged from this service on September 22 -- 1864. He re-inlisted and was enrolled on the 1st day of Novr 1864 in the 1st light battery Ind. artillery and was discharged Augt 22 -- 1865 at Indianapolis. He had been in no service of the U.S. prior to the above date May 12 -- 64 and subsequent to Augt 22 -- 1865. [He] had not made application prior to Oct 7 1892 for pension.

Mulford1, aged 64 years, citizen of the town of Sharon, Walworth County, Wisconsin well known to me to be reputable and entitled to credit, and who, being duly sworn, declares in relation to aforesaid case as follows:

I am well acquainted with Alexander Franklin McFann, during the fall of 1892 and for a year thereafter he lived with Geo W. Pierce on his farm doing light work, he always complained of Rheumatism and kidney trouble, and could not be excited owing to the condition of his heart, he also had a bad throat and claimed it was due to exposure while in the service, he was far from being a well man, and his physical condition debarred him from earning much wages, the disease seemed to have settled all over him.

On this 13th day of April A. D. 1899, personally came before me, a Notary in and for the aforesaid County, duly authorized to administer oaths George W. Pierce1, aged 45 years, a resident of Sharon in the County of Walworth and State of Wisconsin whose Post-Office address is Sharon, Walworth County, Wisconsin well known to me to be reputable and entitled to credit, and who, being duly sworn, declares in relation to aforesaid case as follows:

I am well acquainted with Alexander Franklin McFann, during the years 1892 and 1893 he worked for me on the farm doing light farm work, he not being able to do any heavy work on account of his disability, he was at that time taking medicine for his kidneys and heart, he had the catarh bad at the time and was unable to do heavy manual labor, he complained frequently of his heart and any undue excitement prostrated him so that he was unable to work for quite a time afterwards, he was also badly used up with Rheumatism and could not do a days work, so I kept him looking after horses and doing light chores.

His Post-office address is Sharon, Walworth Co, Wisconsin
he further declares that he has no interest in said case and is not concerned in its prosecution.

1. For information on the connection between E. A. Mulford, O. A. Mulford, George W. Pierce, and Alexander F. McFann, see David A. Heiden's reply to my query on the Walworth County, WI Queries board. Heiden has traced the family history of these Mulfords back to the 18th century. As I understand it from his work, the connection between these individuals is as follows: E. A. Mulford was Ezra Ames Mulford Jr (b. 1835). His (adopted) daughter Anna M. Mulford was the wife of George W. Pierce. O. A. Mulford was Orrin Ames Mulford (b. 1860), son of Delos W. Mulford. Delos W. Mulford and Ezra A. Mulford Jr were grandsons of Ezekiel Mulford (b. 1764), who was also the great grandfather of Sarah E. Mulford, wife of Alexander F. McFann. 2. Michael Cook (1837--1913) was the son of Christian Cook and Elizabeth Bowers. He was a neighbor of Asa B. McFann in Liberty Mills and submitted an affidavit in support of Asa B. McFann's pension claim. Cook served in the Civil War in Co 47 Indiana Volunteers. Marinas K. Martin (1844--1918) was the son of John A. Martin and Amanda McLean. He served in both Co G 138th Infantry and the 1st Indiana Light Artillery Battery with Alexander F. McFann. His sister, Alice Jane Martin, married Michael Cook. Sanford Charles Martin, who submitted the 1899 affidavit with Michael Cook, was the brother of Marinas K. Martin. The information on Michael Cook, M. K. Martin, and S. C.
